Is Physiotherapy the Right Starting Point for You?
If the problem is pain, stiffness, weakness or unsteadiness that has not settled on its own, physiotherapy is a reasonable first step, and you do not need a referral to find out.
- Book a Physiotherapist if: you have a recent injury, pain that limits what you can do, a joint that gives way or catches, stiffness after surgery, or pain that has outlasted the time you expected it to take.
- Book an Exercise Physiologist if: you are managing a long-term condition where exercise is part of the treatment, you are rebuilding strength after a hospital stay, or you have been told to exercise but not told how.
- See your GP first if: the pain followed a significant fall or accident, you have unexplained weight loss or fever alongside the pain, you have new numbness or weakness in a limb, or you have chest pain on exertion. These need medical assessment before an exercise program starts.
- You may not need us at all if: the problem started in the last day or two, is settling on its own, and does not stop you doing what you normally do. Give it a week.

What Funding Can Hornsby Clients Use?
Medicare Chronic Condition Management Plans
Up to 5 Medicare-subsidised allied health visits per calendar year for conditions lasting six months or more. Physiotherapy and exercise physiology both qualify. Since 1 July 2025 the GP Chronic Condition Management Plan has replaced the earlier GP Management Plan and Team Care Arrangement (the old EPC plans); existing plans stay valid until 30 June 2027.

What Happens at Your First Appointment
- Conversation first. Your history, your goals, what you want back.
- Physical assessment. Movement, strength and specific testing.
- Treatment the same day. Manual therapy and your first exercises.
- A written plan. Session estimate, home program, funding confirmed.
What Should You Bring to Your First Appointment?
- Your GP referral or care plan, if you have one.
- Any imaging reports. The radiologist’s written report is more useful to us than the scan itself.
- Medicare card, DVA card, NDIS plan details or claim number.
- A list of your current medications, including anything you take for blood pressure or blood thinning.
- Clothing you can move in, and the shoes you walk in most.
- If someone helps you day to day, bring them. A second set of ears is worth a great deal in a first appointment.
For a home visit, we need a clear space roughly two metres square and a sturdy chair. Nothing else.

What we accept.
Referrals for physiotherapy and exercise physiology under GP Chronic Condition Management Plans, DVA, NDIS, NSW workers compensation and CTP, plus private referrals and self-referrals.
What to send.
The referral, the relevant history, and the funding pathway if one applies. For post-surgical patients, the operating surgeon's protocol and any weight-bearing or range-of-motion restrictions.
What you get back.
A written report after the initial assessment and at the points each funding scheme requires. For care plan patients that means a report after the first and last service, which is what Services Australia requires of the provider.
