Which Funding Pathway Applies to You?
Most Blacktown clients pay for physiotherapy through one of five pathways, and iRehab accepts all of them.

What Can Your NDIS Plan Actually Pay For?
Physiotherapy and exercise physiology are usually funded from Capacity Building supports, most often under Improved Daily Living, and iRehab is a registered NDIS provider (4-3LLM-2259) so both self-managed and agency-managed participants can use us.
If your plan is Agency-Managed (NDIA-Managed)
You need a registered provider. iRehab is registered, so this is straightforward.
If your plan is Plan-Managed
Your plan manager pays our invoice. Send us their details and we deal with them directly.
If your plan is Self-Managed
You can choose any provider. We invoice you and you claim through the myplace portal.
Where in the plan the funding sits
Physiotherapy and exercise physiology are usually written under Capacity Building, Improved Daily Living. Some participants also have Improved Health and Wellbeing funding, which is where exercise physiology often sits. If you are not sure which line applies, send us the plan and we will read it with you.
If Physiotherapy is not in your plan yet
It can be requested at your next plan reassessment, and a therapy report supports that request. Talk to your Support Coordinator or your Local Area Coordinator (LAC).

Sophia is especially passionate about helping her clients rebuild confidence, improve quality of life, and regain independence through movement. She believes strongly in the power of education and support — ensuring every person she works with feels heard, understood, and encouraged throughout their health journey. If getting to a clinic is the barrier, iRehab Physiotherapists and Exercise Physiologists visit homes, aged care facilities, retirement villages and workplaces across Sydney. Therapists bring the equipment needed for each session.
Appointments That Work for the Whole Family
Blacktown is one of the most linguistically diverse local government areas in New South Wales, and a good number of the enquiries we take come from an adult child arranging care for a parent rather than from the patient themselves.
Family members are welcome in the room. For an older client, or anyone who would rather have a second set of ears, bring whoever helps you. We will explain the plan to both of you.
Arranging care on someone else’s behalf. You can call, describe the situation and get a straight answer about whether we are the right service, before anyone commits to an appointment. If we are not, we will say so.
Your First Visit
- History and goals. The injury or condition, your work, your funding.
- Assessment. Movement and strength testing; diagnosis without imaging in most cases.
- Treatment. Hands-on care and your first exercises, same day.
- Paperwork handled. Claims, referrals and reports managed with your GP and insurer.
What Should You Bring, Depending on How You Are Funded?

Who Does the Paperwork, and When
This is the part most clinics leave vague, so here it is plainly.
Before your first session. We confirm your approval, referral or plan funding with the payer. For WorkCover and CTP that means contacting the insurer. For NDIS it means confirming the funding line and, if you are plan-managed, setting up billing with your plan manager. You do not need to do any of this.
At the first session. Your therapist agrees functional goals with you in writing. For claim-based treatment those goals become the basis of the reports the scheme requires.
During the program. Allied Health Recovery Requests for WorkCover, progress reports for CTP insurers, and therapy reports for NDIS plan reassessments are written by the treating therapist. For Medicare care plans, the provider reports back to your GP after the first and last service, which is what Services Australia requires.
If something is refused. Insurers sometimes decline a request for further sessions. If that happens we tell you the same day, explain the reason given, and set out the options rather than leaving you to work it out.
For Support Coordinators, Plan Managers and GPs in Western Sydney
Referrals accepted for physiotherapy and exercise physiology under NDIS, Medicare Chronic Condition Management Plans, NSW workers compensation, CTP and DVA, plus private referrals.
For support coordinators. Send the participant’s plan or the relevant funding pages, goals, and any behaviour support or manual handling considerations. We will confirm capacity and a first appointment before you commit the participant to anything.
For GPs and specialists. Send the referral and history. For post-surgical patients include the surgeon’s protocol and any weight-bearing restrictions. Reports come back after the initial assessment and at each scheme’s required points.
Service agreements. iRehab works with aged care facilities and disability accommodation providers under service agreements for recurring visits.
